We have an exciting opportunity for fellowship at West Road Medical Centre. This fellowship is open to any GP within their first 12 months after gaining their CCT. The GP Fellow will benefit from the experience of other GP Fellows who worked with us on interesting projects (working with homeless patients and women's health) and who are now embedded in our supportive team.
The successful applicant will have one session per week paid protected time (pro rata) for the fellowship. We are keen to co-design this based on interest area and population need. Examples of areas of focus might be exploring socially based models of addressing mental ill-health and emotional distress; working with patients, professionals and community groups to improve the care of those with persistent physical symptoms; better meeting the needs of young people within general practice, working with frail housebound elderly and/ or care home patients; diabetes management – possibly to focus on the differing needs within our population taking into account wider determinants; making care more in tune, integrated and accessible for patients where ethnicity and other cultural traditions might be health determinants, specifically Pakistani Bengali and Muslim traditions. These suggestions are illustrative, we would welcome other ideas.
There is opportunity to contribute and lead on areas of service design and clinical pathways. We will link the Fellow with peer group support and mentoring programmes and offer a GP mentor for professional development.
We are engaged members of an active PCN and collaborate widely with other parts of the local system. We would hope that the Fellow would be able to make use of networking and collaboration opportunities to learn more about their chosen field and drive their work forward.
We have built a supportive structure around the GPs in the team that includes Physician Associate, Advanced Nurse Practitioners, Social Prescribers, Pharmacy Technician, Health and Wellbeing Coach, Practice Nurses, Health Care Apprentices, Care Coordinators, and administrators. We have excellent links with the voluntary sector and we have link workers for children and young people's mental health, patients with learning disabilities, adult mental health and those with long term conditions. The practice boundary includes areas with high levels of health inequalities and health need and provides opportunity for primary care to have a high impact which makes a very rewarding environment for our team and GPs.
