PCN Name: Gosforth Jesmond Health (GJH)

Gosforth Jesmond Health (GJH) is a company set up by the Primary Care Networks (PCN) of North Gosforth and Jesmond and Lower Gosforth to manage and deliver PCN contracts and services. We are innovative in our approach and our working environment is dynamic and complex, as we endeavour to support the six GP practices of our two networks deliver highquality primary care services in challenging times

Job Summary and Main Duties

We are looking for two ARRS eligible GPs to join our PCN team. Gosforth Jesmond Health (GJH) covers two PCN populations of approximately 75k patients in total.

This role will focus on same day care and home visiting so an interest in elderly care is desirable. We run a ‘same day access’ (SDA) service, with a mix of GPs, ANPs and a Paramedic and we wish to add to that team. This is an exciting opportunity to be part of a bigger team, be involved in the development and implantation of new projects as well as build relationships with a number of practices in the Jesmond and Gosforth area.

The main duties of the role will be:

  • Performing home visits across the catchment areas of the practices, working closely with community teams and supporting the holistic needs of our housebound population.

  • Spending time in our same day access hub seeing acutely unwell patients triaged by practices. This service currently runs out of 200 Osborne Road in Jesmond. GJH successfully implemented a GP ‘Winter Hub’ over November ’24 – March ’25 to provide additional capacity for practices and patients and we also run adult and paediatric SDA year-round.

  • Provide GP input into ‘the team’ supporting case discussion and feedback across team members

The visiting aspect is currently being performed by two of the Clinical Directors plus a 1.0 FTE Paramedic Prescriber. We wish to bolster this service in particular to provide a five day a week service with consistent GP input. This service is highly valued by Patients and Practices. It would be expected that the two CDs working into the service would continue to provide some support and mentorship for successful candidates.

Employment Type

This is initially a pilot scheme, as such the roles will be fixed one-year term with the potential to extend depending on the success of the pilot and ongoing funding.
