LMC Templates: Requests from pharmacies to prescribe weight loss medications

Newcastle & North Tyneside LMC have developed a template for practices to use if a pharmacy requests patient information in order to prescribe weight loss medications.


We would, of course, be delighted to be copied in to any such template letters sent, as long as patient details are redacted, in order that we can be kept aware of the extent of the issue.
