LMC Templates: Shared Care Withdrawal

Newcastle & North Tyneside LMC have developed a set of templates to support practices that wish to give notice of their withdrawal from shared care .

Practices are advised that any cohort of patients which are being handed back should be communicated to the head of the relevant department and the medical director of the Trust, the former with a list of the affected patients and the latter with the total number of affected patients.

We would, of course, be delighted to be copied in to any such template letters sent, as long as patient details are redacted, in order that we can be kept aware of the extent of the issue.
