NUTH Interface Reset

At NNT LMC and GST LMC we're trying something new.

The Interface Reset is a rolling programme we're running jointly with NUTH to tackle the interface issues that quietly eat into practice time every day, one at a time.

First, what this isn't: it is not collective action. That remains paused, and nothing in the Reset asks you to do anything outside your contract or to withdraw any service. We recognise it can be difficult, working alone in a practice, to know when and how to push back on requests that fall outside general practice's contractual responsibilities.

Each month, for six months, we'll focus on one common issue. We will set out who should be doing what, and give you agreed wording to use when work lands in the wrong place, so we can all stand together and respond with confidence.

Our first focus, from 1 October 2026, is fit notes. Where a patient's initial need for a fit note arises from hospital care, that responsibility sits with the treating clinician to provide it for the expected length of the illness. No patients should be directed towards primary care for the sole reason of obtaining a fit note after a hospital attendance.

Attached you'll find:

  1. The Interface Reset proposal in full – the aims, how it will work month by month, and how it links to national Get It Right First Time (GIRFT) priorities.

  2. The October action on fit notes – when they should be issued and by whom, with suggested wording to direct back to secondary care for patients and colleagues when a request should have been handled elsewhere.

  3. The information our secondary care colleagues will be receiving

  4. The slides from our launch event
